I am a preferred provider with Pre­mera Blue Cross, Life­Wise of Wash­ing­ton, and Aetna.

Please keep in mind that many in­sur­ance com­pa­nies will re­im­burse for “out of net­work” ser­vices. In this event, I will pro­vide a state­ment  for you to sub­mit to your in­sur­ance com­pa­ny for your per­son­al re­im­burse­ment.

Payment for services not covered by insurance is due at the time of service.
